For and in consideration of acceptance of this entry and permission to participate in the sanctioned 2004 Junior World Water Ski Championships, hereinafter referred to as the EVENT, described on the official registration, participation or entry form on the reverse side or attached to this agreement, which is acknowledged to be good and valid consideration for the promises and warranties make herein, the undersigned, for themselves, their heirs, successors and/or the minor participant for whom they sign do acknowledge, warrant and agree as follows:

1. The EVENT participant is or will be at the time of the EVENT an Active member in good standing of USA Water Ski who is familiar with and agrees to be bound by all USA-WS rules for safety and/or tournament or other EVENT participation and/or Competition;
2. The participant is competent to safely participate in the EVENT and will be fully responsible for his/her own safety and well being at all times, including the safety and sufficiency of all equipment used in the competition, and also included his/her safety at all times and under all circumstances while at the EVENT site. The skier acknowledges and accepts full and sole responsibility and liability for the operation of and any defects and other hazards in skier’s own equipment. The participant and Parent or Guardian, where applicable, warrant and promise that they will assume full responsibility for the conduct and safety of the participant at all times, whether or not in actual participation and/or at the EVENT site.
3. The undersigned acknowledge and assume the risk of serious injury and/or loss of life from participation in the EVENT and, in connection therewith promise, agree and warrant as follows:
1. To release, hold harmless, protect, indemnify and forever discharge the sponsors and organizers of the EVENT, USA Water Ski, its officials, members and co-participants in the EVENT, the land or site owner, lessor, lessee, public or governmental agency having jurisdiction thereof, and/or the site operator, and any agency that may be assigned to cover the EVENT, their agents, servants, employees and all persons connected with the EVENT, from any and all rights, claims and demands of any nature for any and all loss, damage or injury sustained by the participant or his or her equipment at any time during the EVENT or before or after the EVENT but in connection with it; and further promise and agree never to institute any claim, suit or action at law against any such organization or individual described herein above, other than for claims made against a medical insurance carrier for benefits provided to USA-WS members under, and only to the extent of, any applicable USA-WS group medical insurance coverage;
2. The undersigned is of legal age and competent to enter into this agreement, and/or the parent and/or guardian of the minor participant on whose behalf they sign, and hereby agrees to indemnify and hold harmless to the full extent described in paragraph a) above, those organizations and individuals named or described therein from and against any claim or suit due to injury or damage to the person or property of said minor participant;
3. The undersigned hereby authorize the EVENT organizers, directors, participants or their designee to seek and procure on their behalf all reasonable participant who is or becomes unable to authorize such care, and warrant their financial responsibility for such treatment as is rendered;
4. The undersigned grant to the EVENT organizers and USA-WS the right to make and use any photographs, television or motion pictures taken during the EVENT, and agree that the organizers have the right to control or prohibit advertising material used, own or displayed by a participant at the site during the EVENT.
5. The EVENT organizers and director reserve the right to disqualify the participant and/or at their sole and unqualified discretion, to refuse to allow the participant to participate in the EVENT for any reason they deem just and proper, and in such event the participant will be entitled to the return of the entry fee and no more.

I have read all of the above. I understand it and agree to all terms and conditions and affirm all promises, representations and warranties made by me as a condition and consideration for the privilege of participation in the EVENT. I agree to be fully responsible for my conduct at the EVENT and/or for the conduct of the minor on whose behalf I sign.
